Essay

Write a two-page essay addressing the following questions:

• What are your career goals?
• What are your personal achievements?
• What has your involvement been with Epilepsy Foundation of Minnesota and/or your community?
• How have you addressed any obstacles that your epilepsy or seizure disorder has presented?

The essay must be submitted as an attachment, not copy and pasted into the application.

2 Letters of Recommendation

Ideas for recommenders include a physician, teacher, academic adviser, principal, coach, etc.

Letters may be: 

• uploaded in the application, *
• emailed to Mary Meuwissen at mmeuwissen@efmn.org, **
• or mailed to Mary Meuwissen at 1600 University Ave W, Ste 300, St. Paul, MN 55104. **

* Uploaded documents must be submitted as PDF documents only. Word documents or other editable documents will not be accepted.

** Letters of recommendations emailed or mailed must be received by April 30, 2021.
