Seizure Smart Schools At the Epilepsy Foundation of Minnesota, we are more committed than ever to making all schools in Minnesota and Eastern North Dakota "Seizure Smart." We know that when teachers, students and all school personnel better understand epilepsy, they are more equipped to provide the best possible environment for our students with seizures. We also know that "Seizure Smart Schools" is a TEAM effort - school staff, parents and kids can all make a difference when we work together!

The sample forms linked below provide guidelines for collecting data relevant to educational planning for students with epilepsy and can be adapted to meet individual and institutional needs. Each form requests information critical to appropriate health management planning, which can occur only if the data are used collectively. Click on the links at the bottom of this page for classroom activity sheets to help educate your students!
- Student Interview Form
To be completed by the school nurse through an interview with the student, when age appropriate. The student's understanding of his or her epilepsy will provide valuable information in health management planning. The form should be updated every six months or when changes occur.
- Seizure Observation Record
To be completed by school personnel when reporting a seizure(s) and/or medication side effects. - Seizure Action Plan
Designed to provide basic information about a student's seizure(s), and medication, as well as what school personnel should do for the student during a seizure. It should be distributed at the beginning of a school year, when a diagnosis is made or when a change in health status occurs. At the Epilepsy Foundation we have great books and other materials to use in the classroom to discuss epilepsy and seizures with students. "Managing Students with Seizures" is an online training designed especially for school nurses. This training was developed by the Epilepsy Foundation and the National Association of School Nurses. Click here to access the online training.

PARENTS
- Questionnaire for Parents
To be completed by the student's parent(s). In some cases the school nurse may need to interview the parent(s) to clarify and fill in the information requested. The form should be updated annually or when any changes occur and will assist school personnel in recognizing a student's seizure(s), responding appropriately, and planning.
- Seizure Information Sheet
Designed to provide basic information about a student's seizure(s), and medication. It should be distributed at the beginning of a school year, when a diagnosis is made or when a change in health status occurs.
- Parent Packet
The Epilepsy Foundation has developed a folder just for parents to help make their child's school experience a success. Seizure Smart Schools Classroom Education These free classroom presentations are designed to educate students about seizures and how to properly respond to a classmate having a seizure.

This program is designed to educate elementary school students about epilepsy and seizures and encourages acceptance of differences.

This program designed to educate high school students about epilepsy, thereby creating a more understanding and supportive environment for teens with epilepsy.
