Elam Baer & Janis Clay Educational Scholarship      DOWNLOAD 2012 APPLICATION HERE

Ten $1,000 scholarships will be awarded to students who have shown courage in dealing with epilepsy and seizures, commitment to their education and perseverance in eliminating obstacles that seizures may present.

Scholarship amount: $1000 (Ten awarded each year!)
Application Deadline: April 1, 2012
Awards Announcement: May 1, 2012

Eligibility:
Must have a diagnosis of epilepsy / seizure disorder.
Must be a high school senior or a high school graduate.
Must provide proof of acceptance to a post-secondary academic or vocational program.
Must live in Minnesota or Eastern North Dakota.
Must submit a completed application and two letters of recommendation.

April Schmitz Memorial Scholarship               DOWNLOAD 2012 APPLICATION HERE

April Schmitz graduated from Carlton High School in 2005.  She passed  away unexpectedly at home, in her sleep, after having a seizure on Saturday, May 26, 2007.  While April left us way too soon, her memory lives on in a $1,000 educational scholarship in her name, made possible by April's family, friends and loved ones.

Scholarship amount: $1000 (Two awarded each year!)
Application Deadline: April 1, 2012
Awards Announcement: May 1, 2012

Eligibility:
Must have a diagnosis of epilepsy/ seizure disorder.
Must be a high school senior or a high school graduate between the ages of 17-23.
Must provide proof of acceptance to a postsecondary academic or vocational program.
Must submit a completed application and two letters of recommendation, one letter of recommendation must be from an educator.
Must live in Cook, Lake, St. Louis, Koochiching, Itasca, Aitkin, Carlton, Pine, Lake of the Woods, Beltrami, Cass, Superior or Douglas County.

Selection Panel:
Five to seven member volunteer selection committee to be determined by the Epilepsy Foundation of Minnesota Executive Director.
